The relation between whisper test and a mobile audiometry in auditory screening for the elderly

Resumo

Objetivo: Analisar a concordância entre teste do sussurro e audiometria tonal de triagem por aplicativo em pessoas idosas e a associação dos resultados desses métodos de triagem com as queixas audiológicas obtidas por questionário.

Métodos: Trata-se de um estudo analítico, observacional de corte transversal. A coleta dividiu-se em: coleta de aspectos sociodemográficos e queixas audiológicas; inspeção do meato acústico externo e realização do teste do sussurro e audiometria tonal por aplicativo. A amostra foi composta por 32 pessoas idosas selecionadas por conveniência.

Resultados: Dos 32 participantes, 56,3% apresentaram resultado “falha” em, pelo menos, um dos testes. Na audiometria via aplicativo, a média dos limiares audiométricos encontraram-se maiores em 8.000 Hertz à direita e em 4.000 e 8.000 Hertz à esquerda. A principal queixa foi dificuldade de compreensão em ambientes ruidosos. O coeficiente Kappa de Cohen indicou concordância razoável entre os testes aplicados (0,296).

Conclusão: O teste do sussurro e a audiometria realizada por meio de aplicativo apresentaram desempenho semelhante na triagem auditiva. Verificou-se associação entre os resultados desses métodos e as queixas audiológicas, reforçando a importância de integrar avaliações objetivas e percepções subjetivas na análise da audição de idosos. Os achados evidenciam a relevância da realização de mais estudos que investiguem a aplicabilidade dos testes de triagem auditiva recomendados pelo Integrated Care for Older People no contexto brasileiro.

Abstract

Purpose: To analyze the agreement between the Whisper Test and appbased pure-tone audiometric screening in older adults and to investigate the association between the results of these screening methods and audiological complaints obtained through a questionnaire.

Methods: This analytical, observational, cross-sectional study included the following procedures: collection of sociodemographic data and audiological complaints; inspection of the external auditory canal; administration of the Whisper Test; and appbased pure-tone audiometric screening. The convenience sample comprised 32 older adults.

Results: Of the 32 participants, 56.3% failed at least one of the screening tests. In the app-based audiometric screening, mean hearing thresholds were higher at 8,000 Hz in the right ear and at 4,000 and 8,000 Hz in the left ear. The most frequently reported complaint was difficulty understanding speech in noisy environments. Cohen’s kappa coefficient indicated fair agreement between the two screening tests (κ = 0.296).

Conclusion: Participants showed similar results on the Whisper Test and app-based pure-tone audiometric screening. An association was observed between screening results and audiological complaints, reinforcing the importance of integrating objective screening measures with individuals’ subjective perceptions when assessing hearing in older adults. These findings also highlight the need for further studies investigating the applicability of the hearing screening methods recommended by the Integrated Care for Older People (ICOPE) framework in the Brazilian context.
